What do you mean?  Are we talking about spoken or written English?  

The best way to do this is to put yourself in the company of someone who is really good at it.  But I'm not sure that's what you mean.

the 3 golden rules to language - read it, speak it, listen to it. so read atleast a book a week. it can be a romance or even archie comics, but read! dont be afraid to speak english. if u make a mistake, someone will correct you. and you will learn. watch a lot of english movies and TV programmes. it will help you get used to the different accents, and also help you learn contemporary lingo.

I know what they to improve a foreign language. They stick that person in the middle of that native language location and it forces that person to use that language. So you could try and do something similar. Try going into the middle of a place where they ONLY speak english. Try should help with your communication.

if youre always online, good. try joining the forums and chatrooms... i think you mean the conversational english... you will learn about the slangs and the modern terms there..

or try watching movies, start with children's videos or cartoons, the english has to be simple and well-pronounced if youre more on beginner's level..gradually switch to english-speaking videos that suit your learning progress..

novels and literary works are good but remember the words used in them are sometimes inconvenient for everyday use.

whenever your encounter an english word, try to look for its synonyms and antonyms to expand your vocabulary--a thesaurus  will be of great help.

and lastly, try speaking to yourself in english as much as you can--and silently. =D
