How can I to add a middle name?

I want to add a second middle name that is a family last name, but can also be a girl's first name - Kelly.

(first name) (middle name) (Kelly) (Sullivan)

Is it possible to do this? If so how/where do I change it? How much would it cost? Has anyone done this before? Thanks!!!!!!

  1. You go to the courthouse in the county the child was born in, and request a name change form. there is a nominal fee for it. Then when that is done you go to the social security office and get a new card. Its just that easy. Just like getting married and changing your last name.



    And yes you can have that man names. My children have 4 names for the girls and the boys have 5. its all fine and legal


  2. I'm pretty sure you'd have to hire a lawyer to do it.  He will file all the paperwork for you and I agree that it will cost between $100 - 300, depending on where you live.

  3. i'm not sure how and where, but i no it's possible to legally change your name (i think) have u ever read Stargirl? well the girl changed her first name like every week!

  5. My daughter's birth certificate has her first, middle, my and the father's name last. Yes, there is a fee to change your child's birth certificate. I think it cost $10(or more) per letter where I live (FL).  My friend who did hyphenated her and the father's last name but it will be too long for the child to spell it.
