How can i file an insurance claim without provider info like social and date of birth?

My ex husband was ordered to provide health insurance for our two small children. His new wife then added the kids onto their insurance policy and sent me a card to use. The problem i have run into is that we can't file without her social and date of birth and work info as well. The ex will not talk to me and i can not get any government agency to help either. Where do i need to turn? I guess i could go back to Dept. of Human Services and tell Medicaid that his insurance can't be processed and see what they do.

  1. You need to go back to support court, and have the judge get the info for you, or find them in contempt.

    Or, you can let Medicaid chase after him for reimbursement.

  3. Just because you don't have this info, doesn't mean the claim cannot be submitted. The card you got should have enough info on the subscriber for the provider to submit a claim. Any requests for additional info will go the subscriber.

    Short of that, send the bills to your ex with an explanation of what you need (certified).

  4. Call your ex husband and explain the situation to him.  See if he can file the claim.  If you and the step mother don't get along, I can't blame her for not giving you her ssn. Therefore, see if the children's father can file the claim.

    If you need to, follow up with a letter and any documentation he needs to file the claim.  Keep a copy of the letter for yourself so that if you need to go back to family court  you have written proof that you requested he file the claim.
