    1-800-772-1312

    It takes a long time on hold.

    Tell them you want your SSI account information. They will ask you a few questions, and if the answers are correct they will send you your totals. You can get them every year.

    Or you can go on line to

    www.ssa.gov


  2. The government sends them to you.  I'm sure you can request a copy from the social security administration
