If you have an old ration card you have to apply for transfer of your name. So submit application in white paper with old ration card to Thaluk suppy officer of Neyyatinkara, Nedumangadu or Trivandrum Thaluk as the case may be.

For new card approach the TSO in person and submit application. You will have to produce a residential certificate from corporation for both and then TSO will ask you to appear for digital photographing.
