How can i get my Filipino Passport?

  1. Different countries have different laws.  You should contact the Philippine government office that gave you  your passport and ask them what must be done.


  2. are you in the states ? you need to do a report to the police first of all .to make sure , i imagine you will not mention how long ago you have lost your passport :-(

    Lost Passport

    This is in addition to the requirements for new passports below. All those applying for the replacement of lost passports will be interviewed by a Consular Officer. Processing of lost passport is subject to 30 days waiting period.



    What to bring/mail



    # Police report #

    Affidavit of Loss (describing in detail the circumstances of the loss or theft of the passport

    Any valid photo identification card (driver's license, ARC, etc.)

    Marriage certificate and Report of Marriage (for married women). (Note: Marriages contracted abroad must first be reported to the Philippine Embassy/Post for recognition.)

    Any two of the following:

    Original/certified true copy of non-availability of birth certificate from the National Statistics Office, Manila

    Original/certified true copy of the baptismal certificate

    Joint Birth Affidavit by two disinterested persons. (additional requirements for those presenting item a. above)

    Voter's Affidavit or Voter's ID

    School Records (e.g. Form 137, Transcript of records, diploma)

    Proof of service from the Philippine government

    Government Service Insurance System (GSIS) or Social Security System (SSS) Records

    INS Naturalization Certificate

    Certified true copy of the birth certificates of any of the following: spouse, children, brothers/sister

    Death Certificate of spouse or parents

    Seaman's Book

    Professional Regulatory Commission Identification

    National Bureau of Investigation (NBI) Clearance

    Photocopy all original documents

    Bring all original and photocopies to the Consulate for your interview

    Pay $10.00 for fax/communications fee and $25 for notarization

    After 30 days waiting period, present note to the processor to access record

    Pay $90.00 fee for passport booklet

    #Please enclose a self-addressed thick padded brown envelope with sufficient postage (USPS Express Mail is suggested for ease of tracking). Allow five (5) working days for processing time and issuance. METERED STAMPS and PERSONAL Checks are not accepted.     Release of Processed Passport

    The processed passport will be released to the applicant the next business day, between 2:00 PM to 4:00 PM. If the applicant could not claim the passport personally, he/she could designate a representative to claim the passport on his/her behalf, provided the representative presents a valid photo ID card of himself/herself and presents an authorization letter signed by the passport's owner.
