it looks like it was printed up, cut out , glued on and then shellacked...caps were possibly painted over with silver model paint...

Both of these are quite explanitory and pretty easy looking.  Put jewels, tiny mirrors, favorite pictures, etc on yours.

i think they used decoupage... try googling that. i remember watching aleene's creative living and they did that kinda stuff all the time.  theres a product you "paint" over paper that attaches it to surfaces like wood or glass or in this case a bottle cap.
