  1. call the hospital with the original and ask them to send you one

  2. Call the Vital Records Office of the county where you were born. They will give you specific instructions to order a certified copy of your BC.  There may be a form to fill out, and you will need to send them a check or money order to cover costs.  Do not delay, this may take a few weeks.  You should not need to provide ID. I got my BC from a NY county and they only asked me to certify that I was getting my own BC.

  3. Many states will have a way to order a certified copy of your birth certificate. In addition, there a private companies that will do the same, all charge a fee.

I had to have one a couple of years ago when I went on a cruise.  I ordered one from my state of birth, and to be safe I also ordered one from a companies, Results: the private company had mine to me in 6 days, the state took almost 21, only arriving the day before I left on the cruise.

  4. In Orange County, California, I had to go to the County-Clerk's office and paid I think $18 for a copy, with the official engraved seal on rainbow colored paper and blue ink.

It says "Certified copy of vital records" on the bottom, with a stamp of the date issued (the day I went to get it), a unique bar code, and a copied signature of the clerk-recorder of the county. Then in even smaller print on the bottom it says "This is a true and exact reproduction of the document officially registered and placed on file in the office of the Orange County  Clerk-Recorder." But the funny thing is, for my middle name, they only list the initial whereas the original BC had my full middle name spelled out lol so who knows what other errors are in there :) But now I can choose any middle name I want starting with an M!! It just says M. lol

  5. Dude I found this by google ing it, it took about 7 min.

Oh and if you weren't BORN in CA, you can follow the links I provided to find the state you were born in.

You have to ask the state you were born in for the official document.

The official form for CA to be filled out is here:

    http://www.cdph.ca.gov/certlic/birthdeat...

    And the directions are here:

The initial webpage that gave me the link is here:

  6. i think u go 2 the hospital were u were born n ask them

but im nt a 100% sure

  7. Courthouse should have applications.

In Ontario, if you cannot get a birth certificate, a baptismal cert will do :-)

  8. the easiest thing to do is to go to the hospital where you were born, then ask for another- do your best to bring information to prove you are who you say you are- IE- social security etc.

  9. Contact the Department of Health in New York and ask for a certified copy with their seal.  They'll charge you for it.  In ND it's $7, in NY it's probably more.

  10. here is the web site from new york's state's department of health... i hope this helps:

  11. lookup online your city of birth website, order one online, they cost a little under 20 bux i think, but thats how your gonna get it.

  12. There is no such thing as an official duplicate. You need to contact the courthouse in the county where you were born, usually the county clerk's office. They have a process for releasing certified birth certificates, the kind that you need.

I need to do the same thing, had six of them and gave my last one to an agency last year, now I have another agency demanding a certified birth certificate.

Google the county where you were born, call the clerk and ask for their procedure. It'll cost a few bucks, but you gotta have it. Get three or four copies while you're at it.

  13. Well I live in Washington and for me to get an official duplicate, I have to send a letter requesting it to the county where I was born. It also costs $3.00. You can also call to request one and sometimes order one on the internet. Go to google and search the county in NY where you were born. Type in the county name, NY, and birth certificate and you should get a website for the county's health and social services department, which is usually who keeps the birth certificate records.

  14. Take it to Kinko's...

  15. Contact New York state bureau of vital statistics.  A state official has issue and verify that a copy is certified.  Copying services cannot do this.  Hospitals do not issue birth certificates.  They send the record of the birth to the local and state officials, who issue the actual birth certificate.
