  1. A familiar problem.

    You need to get a crack chaser for a 4 1/2" grinder.V the crack and use a product called ardex cp,it has the best bonding polymers on the market.Ardex can be purchased at an construction supply store,I use whitecap a division of Home Depot.

    You can V the crack and use a self leveling concrete caulk available at Home Depot/Lowe's.Remember this stuff is self leveling and very runny so keep a piece of cardboard real close.

    Both ways are very simple but a pain in the ???.
