Look for an insurance broker in Yellow Pages. Call them if they can't help they will put you in touch with a broker who can. I have had a replacement heart valve and I got an annual worldwide policy for my wife and I for less than £100.

I don't know if they can. You need to contact the insurers that do specialist health insurance. I guess very expensive. For all EU countries - if you are a citizen of an EU country - there is a form obtainable which covers citizens of the community for health care in any other country.
