How did the triangle shirtwaist factory fire start in 1911?

For those who don't know, the fire at that company was the worst workplace disaster in NYC prior to 9/11.  148 women died in the fire.

The cause of the fire was never determined, although it could have been faulty wiring or careless smoking in an environment which was chock full of flamable material.  Saddest of all, the emergency exits were deliberately locked for reasons of the company's security rather its workers.

Possible causes: lit match, or cigarette.

Or faulty electrical wiring.
