How did they get the saying "Ladies First"?

It is from a time of chivalry when it was considered good manners for a gentleman would open a door for another.  This was often done out of respect for parents, clergy, elders, employers, and ladies.  When a group approaches there may be a question of who goes first. Even the boss or the elderly may yield the respect to them and gesture that Ladies go first.  For what would a true gentleman think of himself, if he put himself ahead of the ladies?
