  1. All you have to do is call your old clinic's records office and request they send the records to the new clinic. You will probably have to sign a release of information form giving them consent to do so. Then it might take a few days or even weeks before the clinic sends the records, unless it's an emergency and they need the records right away, then your new clinic can usually call and get them faster.


  2. You have to go to the office you are transferring from with a written request to have your medical records transferred to the new office and you must sign a HIPAA release form.  They will not transfer your records until they have the HIPAA release form on file.

  4. where i live the new dr. has to officially request them. then they get them on-line in mere moments. you personally can never receive a copy. you are allowed to view it, but will never be issued a copy, and obtaining one fraudualently is a serious offence.a buddy of mine printed his out at the doctors office while waiting. used it for a court case and was fined 5grand, and got 2 years probation.

  5. getting all the necessary information can be VERY time consuming and A LOT of work/effort...I suggest once you have it all you store it all yourself at a site like http://www.MedicalRecords247.org - this way when you need the info FAST in the event of an emergency, it can be accessed anywhere you have internet access.....pretty cool..

    Usually it goes by the number of pages. a copy fee they call it

    either call or go in, you will fill out a form and give them the $, sometimes they a lot to copy so you may have to wait a week at the most.

  8. It shouldnt be hard . Depends on the Dr. you get them from. Some charge a copy fee of $10- $25
