How do I apply for a visa to Pakistan?

I need to know where to send the documents if I live in KY and I need to know if I have to send a self addressed stamped return document envelop inside or do I send money to them to mail it back? Please help me and who do I make the money order out to? Thanks the site is not infomative and nobody answers at the embassy or returns my calls. Oh, also one more thing do I have to include a copy of my travel itinerary.

Be persistent in calling the Pakistani Embassy because it is the only choice you have to apply for a visa to Pakistan.  Know the documents that must be submitted in order that your visa will be granted.

Pakistan missions abroad are authorized to grant visit / tourist visas for three months validity and stay with double entry to foreigners of 175 countries, including the United States.
