How do I explain the reason for leaving my last job?

Filling out a job application form I was asked why I left my last job. I was concerned with a back condition which has much improved. How should I write down the reason why I left without sounding too negative?

  1. you NEVER have to say anything about your last job that you don't want to reveal...i always say IRRECC0NCILABLE DIFFERENCES, meaning that there were problems you wish to not speak of and if they ask you about it just say something about like bad pay or not enough hours or that you moved or something you dont have to tell everything and if you did that's what might keep you from getting the job anyway:]

    i lied about the way i left my job and it just ends up coming out at one stage or another.  if you tell the truth you wont always be worried about if there going to find out or not.

    seriously, i think your condition is a very good reason to leave a job.
