How do I fill out the address form on a website to send something to France?

So my friend is going back to France soon and I want to send her something from a website. She gave me her address but she gave me a city and a "village" so I'm not sure what I need to use.

There are spots on the form for address and city. Can someone please explain to me how to fill it out so it gets to the right place?

FAMILY-NAME Street name

# STREET

VILLAGE

POST CODE TOWN

So it could be like this :

DUPONT Jean

1, rue du Général de Gaulle

CROZON

33030 MORGAT
