  1. Assuming you live in the U.S....

    To get a passport for the first time you have to go in person. Luckily there are several options, I was able to get mine at a Kinko's. Many post offices can help and some libraries. You need two pictures, proof of U.S. citizen and a picture ID. You have to fill out an application and there is a processing fee. Looks like it can take as little as 4 weeks and there is also an "expedite" process which may cost a little more.

I promise you...you cannot get a passport at Kinko's!  See site below for all passport application info and to print out the forms.  Cost is $100.00 and it takes under 4 weeks regular processing and under 2 weeks if you pay for expediting and express mail.  See 2nd site to find nearest place to submit your application.

Charlie's answer if very good, so I will add to that....depending on the city you live see passport website, if you are traveling within 2 weeks you may make an appointment and a passport office and get it that day.  ex. you are traveling 9/10 and live in boston....you may call the boston office, make an appointment, come in expect to be there a few hours, and get your passport that day.

  4. You can do it through the post office and I think it costs about $100.  You will need to pay for your photo too, which the post office will do for an extra $15 or so.  You should have it within a few weeks as long as there are no questions or problems.  

    It took me about 2 months because my name is different on my birth certificate than on my drivers license and I had to submit a ton of proof that I actually am me.
