How do I go about changing my first and last name?

I'm getting married next month and will be taking his name. For some time now, I've wanted to (slightly) change my first name (from Jill to Jillian). I figured it would be just as easy to change it all at once. Is there extra paperwork involved? What extra steps do I need to take?

1. I was shot by my ex who is the father of my daughter, he is getting out of prision this year after serving only four years. I am still affraid of him i was thinking of changing my name so he cant find me. I want to know if it cost money and were do i go to start the procese  and how long would it take...
