you should consult your lawyer about the subject. he/she would get the papers done for you, while in the mean time better get a bodyguard. haha

Go to your local courthouse and find the clerk's office.  They have restraining order packets that are free of charge.  File out the documents, file them with the court and serve the other party.

If you can afford an attorney hire one... if not get a competent paralegal in your area to help you with the paperwork.

alert your local police station with evidence of harrassment, they can look into the case for you. If they break the injunction its like an asbo and can result in imprisonment.
