How do I mail a letter to Cusco, Peru?

I need to mail a letter from the US to Cusco, Peru. I have the address, but I'm not sure how to format it on the front of the letter.

The address is

Urb. Constanza 12-B, Wanchaq

I read Wanchaq is a district, so do I write the address like this?

Urb. Constanza 12-B

Wanchaq, Cusco, Peru
