How do I obtain information about my expired and lost passport?

I need to get the passport number and the place of issue from my first passport, which was issued about 14 years ago. It was also issued under my birth name which has since been legally changed and all of my current records (including my second/current passport) are under my new name. Which agency do I contact to get this info?

FYI: I'm a US citizen, and this is for a US passport.

Ask for a copy of your application for a passport that issued it.
