How do I send a form automatically to a person who are interested in joining my groups?

My groups are restricted. Only the people that have certain criteria can join. Before I know whether or not a person has those criteria, I wanna send them a form to fill in, so that I can decide to approve or not to the person

After setting up the form and saving it on your computer, go to your group site's Files area and Add a new file. In that process, you would upload the document from your computer.  There will be a designation to select to send it upon Pending Membership.  It will automatically be sent to people when they are going through the joining process.  

You would just have to keep track of which pending member returns the completed document to you for your approval.  If they don't return it completed, then you can just deny the membership request.
