How do I verify an electic form (for jury duty)?

I'm not sure what you mean.  If you are opting out of jury duty, look on the form for what kind of verification they want.  A couple of years ago, I got a jury notice the day after I was supposed to appear due to an address change.  I had to check the box on the back and took it to a notary, then mailed it in.  Otherwise, there is usually a phone number for questions somewhere on the notice.

Well in Michigan they send a letter to you and on it it states that you're required for duty unless under some circumstance that you're not able to participate there's a number you need to call to verify that you're not attending. Perhaps you should call your local judicial courts or something and tell them. Or if some notification was sent try to find some sort of phone number.
