  1. for yourself your insurance carrier sends you a certificate you're supposed to have in the car at all times (assuming you're paid up on the premiums; otherwise you'll get notices saying it will be cancelled - if the car is being financed they will also notify the lender if insurance is cancelled).

    the details of the policy were given you when you signed up - it's all spelled out for you.  or if you aren't organized with paperwork, call the insurance carrier or agent (if you can find the phone no.)

  4. Your coverage or somebody else's?  If yours, check your declarations page.  If somebody else's you need to contact that person's insurance company assuming that you had an accident.  They will not give you coverage specifics however.
