How do I write this type of email?

So, I'm sending an email to the Brookhaven National Lab FACE Program, to see if I could participate in their program. I'm seventeen, and a high school senior. How would I start this sort of letter? (Heading? What after?) Thanks!

Dear Sir/Madam

I am (Your name) and I'm a high school senior and I would like to participate in your program. Could you give me some information on it?

Write that sort of thing and tell them what ru doing at school (highers or whatever) and why u would want to do it.

Sign it off with "look forward to hearing from you, and yours sincerely and then ur full name at the bottom.
