  2. There's no blood test. It's a questionnaire that determines how distracted you are in different situations.

  3. There is no definite, clear cut test. The evaluation basically consists of a questionnaire. Unfortunately, this means that the diagnosis is somewhat subjective and it is possible and really common to fake ADD just to get a RX for a narcotic like Ritalin/Concerta/Focalin/Vynase/etc.

  4. A good evaluation is more than a questionnaire.  My son had about 6 hours of evaluation, including standardized testing, interviews, a video game sort of test that looked for impulsive behavior, and observation before he got his diagnosis.

    I'd recommend finding a psychologist who concentrates (pun intended) on ADD.  If medication is recommended, a pediatrician can use the psychologist's evaluation as a basis for treatment.
