How do i add a middle name to myself?

I was born and live in New York. And i was never given a middle name, i want to add one, what do i have to do?

Im 17, so i know my mom has to be with me with whatever i have to do

  1. Honestly, why bother?  You never use a middle name anyways.


  2. legal name change.  yes at 17 your legal parent or guardian must go with you to the court house to file for this being your a minor

  3. Hey! Well actually I got my own middle name when i was 11. I wanted my cultured name to be a part of my official name. So First, we had to fill in a form and send it (you can find this online)

    Then you gotta go to court and everything, and you gotta pay like $300 (about). By the way the court part is not like weird you just need to go there so that the guy can stamp your form.

    Then you have to change your passport...

    I like my new name:)

    and you can go to your local county court-house to make it official~!!!

  8. An easy answer is to get a AKA (also known as) or a DBA (doing business as) at your county court house.  Then you can choose a middle name and have a right to use it legally with banks or whatever.

  9. I know how you feel, I don't have a real middle name either, the people that did my birth certifacate forgot to put my middle name so legally I don't have a middle name
