How do i fill abandonment of a child

Ok i i am in midvale utah and i want to know how do i file abandonment of a child on my wife she left us 3 months ago

Are you charging her with a crime?  Then call the cops.  Are you using it to get legal custody?  The talk to a lawyer.

You would not file "abandonment"... you file for divorce, and SOLE custody.  Since I just put this question together with your other one.. if wife abandoned you, then you need to be sure that if you ALLOW any time with the inlaws  ... that it be supervised in your home, or somewhere else.  I'd say you have a strong case to suspect that someone is likely to run with the child.  

If you are not comfortable with court... please don't risk trying to do this, without a lawyer.  You might not want divorce, but unless you go to court, and get custody orders quickly, it puts you at great risk.  In some states.. a parent who leaves the state with a child BEFORE custody orders, is free to do so. In other states, it would be parental abduction, court orders or not.   The rules are different from state to state.  If you don't have any lawsuit filed.. you are open to possible problems.
