How do i fill out form 4506-T?

I need help filing out the 4506-T form. All i need is the proof of non-filing. I was wondering should I put my social security number? and should i sign it on the bottom? It said don't sign it if i don't have 1a or 1b. Confusing!

  1. The owner of the Social Security Number must sign the form 4506-T, and IRS will match the signature with their files.  And if they don't match then the information will not be mailed out.


  2. Like several of us told you earlier, *YES*, you have to put your name and your social security number in section 1 of the form. If you don't identify yourself on the form, the IRS will not know who is making the request.

    I don't understand what you mean when you say "It said don't sign if I don't have 1a or 1b". I don't see anywhere on the form that says anything like that. It *does* say that the form must be signed by the person whose name is listed in line 1a ... that would be you.

    All you do is put your name in 1a, SSN in 1B, check box 7, and then sign your name at the bottom.
