I bought this great book on Amazon called "Wedding Vows & Traditions" by Cathy Howes.  Its white leather bound (almost looks like a bible) and is just brilliant, really great verses & ideas in there, for all types of religious/civil ceremonies.  I'd definitely recommend it.

They have TONS of vows in their bulletin board -- it's a vow repository, and the ones we used to base our vows on.

When you give notice at the registrar's they will give you your options, but alot of website have them, google them.

Don't know I'm afraid - but interested to see your replies.  (May be a church website or something would help).

I know someone who wrote their own vows.  All very personal and meaningful.
