How do i find out my daycare identification number .my child is no longer enrolled and she has never given it?

  1. If you have paid her money to child care, she is required by law to provide  you either her social sec number or her employer identification number if she has one.  If she refuses, I would download for W-9 from the IRS and give it to her.  It says right on it what it is for (not necessarily daycare, but to give to anybody to whom you pay money) and maybe the fact that she is looking at an official IRS form will convince her to come across.  With all the identity theft, people are careful with their SSNs so maybe she is just being careful, and does not understand she is required to provide it.

  2. Go to IRS.GOV and download form W-10 (that's the correct form).

    If she still doesn't give you the number, when you file next year you will attach a note to the form 2441 explained your efforts to get it from her.
