Who reports gambling winnings? what will you do when you continue to gamble and lose all that money, and your total winning becomes negetive?  you will  then have payed a courtesy tax, complements of you being stupid and reporting gambling winnings in the first place.  If your not playing  with an advantage it is 100%** certain that this will happen at some time in the future if you continue to gamble indefinately**

but if you must you can contact the casino and get a win/loss statement

You need to contact the organization you won the $$$ from.  If it was enough of a win to warrant a W2G and they are required to follow US tax law, they will resend your documentation.

if you use a players club card at a casino just contact them and they will be happy to send one to you otherwise i have no idea
