i was born at holy cross hospital in cook county.my parents were richard otto storheim and lorraine june storheim.mothers maiden name is kammien. i was born in 1959

  4. You  go to the Hospital that u were born at in Chicago Illinois,or look  up information on the internet how to obtain your birth certificate. 1 u know your parents name. 2 u know when u were born the year. 3 u know your name so the Hospital should still have your document when u were born.

Go to the Cook County Courthouse under records.  You can do an online search if you need to.  I think because you are ordering for yourself that you have to go there in person to show proof of who you are, like mail with your address on it, a social security card or Drivers ID.  Call ahead and ask them exactly what is needed.  It should cost about $15 dollars to get and you should be able to get it that day.

  21. Call the state health office in that state. They will help. You shouldn't give personal info on the web like this. There are a lot of crazy's out there stealing peoples identities.
