for state check call the department of revenue in hawaii

For the federal check, call the IRS at 1-800-829-0922 and report that you lost it. They will initiate steps to get your a new one.

For federal you can fill out form 3911 (download from the IRS website) and mail to appropriate address.  IF you call IRS they will just tell you that they will send you the form or tell you to download the form yourself.
