  1. all you have to go to get your name changed is the police station... i think...

  2. contact your vital statistics agency.. They will send you the information to do so.. you may have to get permission from the father to do so cause he may have to sign the document giving permission to change the name.

im going through the same thing well not really but I want to have my daughters dads name added to her birth certificate.but he doesnt reside in canada so its alot of bs to go through with getting stuff notarized n stuff..

  3. get lawyer or some shittttt :))) good luck !! :)))000

  4. It really is quite simple, went throught it with my oldest. You will need a lawyer, if I remeber correctly it was about $1200. However, you get the lawyer, fill out all the paperwork for a legal name change, the will then run an ad in a newspaper to announce this, the father may be able to object but i wouldn't worry too much about it if you have not een him in a while, you then go to court, judge asks you couple questions, why do you wanna do this etc? May even ask your son a few ?'s my daughter was 5 and got asked do you want to change your last name? When she answered yes, I want to have he same name as my mom and dad and baby brother, whole courtroom started crying and boom it was done.  Good luck!

  5. You go to the same court that people go to for a divorce. You can probably go to city hall and ask the city clerk if there's a form so you can do it yourself rather than have to use an attorney.

  6. Inquire  at your city courthouse records of birth There will be a fee but it could easily be done

  7. you have to file court papers to legally change a person's name.
