How do i get past medical records from my old doctor?

i have a new doctor and i need to get all of my old records from my old doctor, i don't even know where to begin.

Your new doctor can give you a release of information form to fill out and send to your old doctor, giving them permission to release your records to your new doctor.  

Or you can call your old doctor and try to get a similar form so they will send your records to your new doctor.
