How do i list this on a college resume?

im a senior in highschool, and my teacher is having us make up our college resumes with our classes and activities. She told us to include if we played an instrument. it was a few years ago, and im not really sure how to list it...should i just say 2000-2004:piano?

its not like i did it through an orginization or for school. so should i even list it?

Don't put the dates.  Just say that you play the piano.

It isn't like the school is going to make you do a recital to see if you can still play.
