how can i make one that looks like this? ^

not just a regular box, and button below,

but make like any shape on photoshop then slice the image to make a text box inside of the image to make a comment box?

It really depends on what programming language your using. Javascript, php, java... I know that if your using php you should be able to use an image to replace a button but you have to create a rollover version of the image as well.
