sometimes my cutomers write checks on my dba name and the banks return my checks

There are two things you must do, first go to the city hall where your business is based and apply for a business license as a sole proprietor doing business as, what ever the name is the cost is usually under $75. Next go to a bank of your choice and open a business checking account, the bank will tell you the necessary steps, you will need to file a fictitious name statement with a local newspaper which is very easy.

Yes, banks will do this.

you need to establish a DBA with either a local govt group or the state.  

If you provide this to the bank you will be clear.

You need contact the group in your state that that handles incorporations.  Every state is different.
