    You can fill out form and then print completed form with your printer. I normally make a copy for myself and file it for my records.

    Form questions:

    Parte I: Inscrito: name of person on birth certificate

    1. Apellido Paterno: your paternal last name

       Apellido Materno: your maternal last name

    2. Date of birth; day/month/year

    6. why do you want your birth certificate?

    REMEMBER to sign and date form, I once forgot to sign it and they sent it right back; I had to sign it and mail it again.

    Mail form with:

    a) $5.00 postal money order payable to:

    Secretario de Hacienda

    b) self addressed business (long) stamped envelope

    (they'll use the envelope you sent with your address and a stamp to send you your birth certificate)

    c) a black and white copy of your driver's license, or passport

    *call them to see what other forms of current picture identication they accept

    IF you were born AFTER June 21, 1931

    (if you are younger than 77 yrs old)

    IF you were born PRIOR to June 21, 1931

    you must mail your packet to the local town you were born in.(77 years old or older)

    All this information is printed on the form.
