How do i patch up cement block chimney? need help..got pics?

well here's the lowdown. we bought our house about 4 yrs ago and have been fixing it up and we finally got around to the side that has a concrete block chimney that is attached to the furnace..it looks like a hack job..now this house is pretty old so it may just be age....but i'm not a mason so i could be wrong. we just want to patch it up for now and then later my wife wants to have a mason come over and make it look like an actual chimney so it's asthetically pleasing. well i took 11/12 pics so you can see what i'm talking about. Here's my list of things: 1. it doesn't look straight 2. you can see it leaning away from house and a huge gap is evident 3. trim was placed in between the shingles and the chimney..is that normal?? is there something else that can be placed in between that would do a better job.. 4..i actually had to put cement at the bottom because there was a hole going into the house. what should i do to patch it up until i got money to get a new one?

  1. I would suggest stucco but I'm not sure the chimney could handle the load  it needs to be torn down build a proper footer under it and have a reputable mason redo it
