How do i sign up for unisun an alternate insurance when i have medicaid?

i have a medicaid card but it only allow me to use certain services concerning dental insurance and i would like to be able to use other services under unisun's dental plan so how do i sign up for unisun which allows medicaid patients to receive more benefits.

They have a form that needs to be filled out that entitles you to additional benefits, you must phone them and request it
