  1. Simple:

Step One: Teach your dog to lay down and teach your dog the stay command. Hold a treat in front of her nose and lead her into a down. You can push gently on her shoulders if you want. For stay just tell her "stay" and give her a treat if she doesn't move.

Step Two: Once in a down position lead her nose to one side. Gently push at her nose with the treat until her opposite shoulder is on  the floor. Praise.

Step Three: Lead her head onto the floor.

Step Four: Increase the time you keep her flat like that. Put her in a stay. Gradually move yourself away from her. Praise.

Step Five: Add a command like "bang" and lead her into the dead position. Wait five seconds. Praise.

  2. shoot him

  3. I got a book called 101 Dog Tricks by Kyra Sundance and Chalcy.  It it totally wonderful.  I don't want this to sound like a commercial but my Tanner 10 and Bruiser 6 didn't even know any commands but sit.  Now they each know about 10-15 tricks I've not gotten to playing dead it's an advanced trick but I know it's in there.
