How do i transfer my cousin out of a school to another school closer?

The problem is we don't have a shot record and birth certificate only a Social security number. The school he is in is far away from where we live and his mother gave temporary custody to my mom. we need to get him into a closer school.Need help ASAP school will be starting soon in KCMO.

His present school may have those missing records. In any event show his new address and use that to register him into the closer school../
