How do i wire headphones up to a stereo radio?

alright, ive got a dodge conversion van and a stereo radio (left and right channel) and i want to hook it up to the headphones in the back. The headphones are standard Left+, Right +, and ground. The radio is Left+, Left-, Right+, and Right -. The way it WAS, where it never worked... right+ to right+, left+ to left+, and the vans ground to the speakers -. can someone clarify this to me of why they did it and how it should be?
