How do people usually get backstage passes to concerts?

Question

Whenever I go to a show I see those few lucky people with backstage passes, and I wonder to myself "How the heck did they get that?"

Besides winning a radio contest or knowing the band is it ever possible to purchase a backstage pass? I know this may sound ridiculous but I have no idea, that's why I'm asking!
