They are prepared at a catering company, usually near the airport and delivered prepacked as individual meals not unlike a TV dinner.  The are packed into rolling carts and put on board the aircraft.  Hot meals are transferred to an on-board oven before serving.  The airline can specify what meals they need for a specific flights. They can pre-order special dishes for flights to certain destinations as well as vegetarian or kosher meals.

    Over the years they've developed recipes catering to the preferences and tastes of the majority of passengers.  On domestic flights they serve meals that are  somewhat less spicy than international flights or flights to some exotic or holiday designations.

    Airline food has gotten a bad reputation over the years but catering to the tastes of millions of travelers is not an easy task. It's all done behind the scenes by hardworking people trying their best to deliver a good product.
