How do wedding gown boutiques work?

Question

Say I really like a strapless dress but want a different neckline and sleeves with the dress.

How does that work? Do they order the strapless for me or send the requests with the dress?

Well, it sounds like you are looking for a custom dress.  If you try on a dress that you like but want to change it significantly, you would either need to pay to have it altered (if possible), find another dress like it but in the style you want, or take a picture of the one you like to a seamstress and have her create a blend of the two.  A gown boutique is similar to any other store, they can order you what the designer makes or recommend things, but they can't magically make the dress you want.
