  1. Basically, match the colors.  The little plastic thing at the end of the phone cord is a quick disconnect and has to be inserted into a wall jack with a corresponding receptacle.  Wal Mart, Home Depot, Ace Hardware all carry telephone supplies.  Get a new wall jack and replace the old one.  Again, match the wire colores.

  2. As you said there are 4 wires on the wall. They should be black, green, red, and yellow. Just get the green and red wires. Connect these wires to your cord phone line. These wires can be reverse, so you don't have to worry about it.

If the wall jack has different wire colors, just find a pair that can produce a dial tone when you connect them to your phone. Do trial and error. Don't worry you wont get shocked unless someone will call your number. The ringer signal has a little shocking current.
