  1. Are you discussing CREATING a train?  If so-

    Let's say this -- at, so, 5 am, an engineer gets a call from the crew caller; "dude, you got a train to make up."  So the engineer heads to the train yards.  There is the switch engine warming up for him to use.

    He [she] hops in, gets the train manifest from the yardmaster, and proceeds to pick the cars from their respective tracks.  [this is done at a FLAT yard only; usually, at a hump yard, it's set up where the entire train is on one track, ready to pull.]

    Once the train is set together, then the main line crew backs on to the train, pumps up the air, checks on the "Japanese Conductor", grabs the manifest, and off they go.

    Meanwhile, back at the switcher, they're setting up another train.

    If you're looking for a definition, a train consists of a lot of individual cars, and some form of motive power, so either locomotives or multi-units (in which some cars are powered, and some aren't).
